Helium or neopentane can be used as surrogate gas fill for deuterium (D2) or deuterium-tritium (DT) in laser-plasma interaction studies. Surrogates are convenient to avoid flammability hazards or the integration of cryogenics in an experiment. To test the degree of equivalency between deuterium and helium, experiments were conducted in the Pecos target chamber at Sandia National Laboratories. Observables such as laser propagation and signatures of laser-plasma instabilities (LPI) were recorded for multiple laser and target configurations. It was found that some observables can differ significantly despite the apparent similarity of the gases with respect to molecular charge and weight. While a qualitative behaviour of the interaction may very well be studied by finding a suitable compromise of laser absorption, electron density, and LPI cross sections, a quantitative investigation of expected values for deuterium fills at high laser intensities is not likely to succeed with surrogate gases.

The last interglacial, commonly understood as an interval with climate as warm or warmer than today, is represented by marine isotope stage (MIS) 5e, which is a proxy record of low global ice volume and high sea level. It is arbitrarily dated to begin at approximately 130,000 yr B.P. and end at 116,000 yr B.P. with the onset of the early glacial unit MIS 5d. The age of the stage is determined by correlation to uranium–thorium dates of raised coral reefs. The most detailed proxy record of interglacial climate is found in the Vostok ice core where the temperature reached current levels 132,000 yr ago and continued rising for another two millennia. Approximately 127,000 yr ago the Eemian mixed forests were established in Europe. They developed through a characteristic succession of tree species, probably surviving well into the early glacial stage in southern parts of Europe. After ca. 115,000 yr ago, open vegetation replaced forests in northwestern Europe and the proportion of conifers increased significantly farther south. Air temperature at Vostok dropped sharply. Pulses of cold water affected the northern North Atlantic already in late MIS 5e, but the central North Atlantic remained warm throughout most of MIS 5d. Model results show that the sea surface in the eastern tropical Pacific warmed when the ice grew and sea level dropped. The essentially interglacial conditions in southwestern Europe remained unaffected by ice buildup until late MIS 5d when the forests disappeared abruptly and cold water invaded the central North Atlantic ca. 107,000 yr ago.
Cotton Stage of Growth Determines Sensitivity to 2,4-D

The anticipated release of EnlistTM cotton, corn, and soybean cultivars likely will increase the use of 2,4-D, raising concerns over potential injury to susceptible cotton. An experiment was conducted at 12 locations over 2013 and 2014 to determine the impact of 2,4-D at rates simulating drift (2 g ae ha−1) and tank contamination (40 g ae ha−1) on cotton during six different growth stages. Growth stages at application included four leaf (4-lf), nine leaf (9-lf), first bloom (FB), FB + 2 wk, FB + 4 wk, and FB + 6 wk. Locations were grouped according to percent yield loss compared to the nontreated check (NTC), with group I having the least yield loss and group III having the most. Epinasty from 2,4-D was more pronounced with applications during vegetative growth stages. Importantly, yield loss did not correlate with visual symptomology, but more closely followed effects on boll number. The contamination rate at 9-lf, FB, or FB + 2 wk had the greatest effect across locations, reducing the number of bolls per plant when compared to the NTC, with no effect when applied at FB + 4 wk or later. A reduction of boll number was not detectable with the drift rate except in group III when applied at the FB stage. Yield was influenced by 2,4-D rate and stage of cotton growth. Over all locations, loss in yield of greater than 20% occurred at 5 of 12 locations when the drift rate was applied between 4-lf and FB + 2 wk (highest impact at FB). For the contamination rate, yield loss was observed at all 12 locations; averaged over these locations yield loss ranged from 7 to 66% across all growth stages. Results suggest the greatest yield impact from 2,4-D occurs between 9-lf and FB + 2 wk, and the level of impact is influenced by 2,4-D rate, crop growth stage, and environmental conditions.

We have developed high damage threshold filters to modify the spatial profile of a high energy laser beam. The filters are formed by laser ablation of a transmissive window. The ablation sites constitute scattering centers which can be filtered in a subsequent spatial filter. By creating the filters in dielectric materials, we see an increased laser-induced damage threshold from previous filters created using ‘metal on glass’ lithography.

Background:
Studies using acute tryptophan depletion (ATD) to examine the effects of a rapid reduction in serotonin function have shown a reduction in global cognitive status during ATD in Alzheimer's disease (AD) and Parkinson's disease (PD). Based on the severe cholinergic loss evident in dementia with Lewy bodies (DLB) and Parkinson's disease and dementia (PDD), we predicted that a reduction of global cognitive status during ATD would be greater in these conditions than in AD.
Methods:Patients having DLB or PDD underwent ATD in a double-blind, placebo-controlled, randomized, counterbalanced, crossover design.
Results:While the study intended to test 20 patients, the protocol was poorly tolerated and terminated after six patients attempted, but only four patients – three with DLB and one with PDD – completed the protocol. The Modified Mini-Mental State Examination (3MSE) score was reduced in all three DLB patients and unchanged in the PDD and dementia patient during ATD compared with placebo.
Conclusions:This reduction in global cognitive function and the poor tolerability may fit with the hypothesis that people with dementia with Lewy bodies have sensitivity to the effects of reduced serotonin function.

Phosphatized and phosphatic small shelly fossils are a major source of information concerning the evolution of animals during the early Cambrian. Although progress has been made in understanding some of these fossils, many remain enigmatic, both with regard to their phylogenetic affinities and the overall morphology of the animal from which isolated sclerites came. Two unusual fossils from the upper lower Cambrian (Qiongzhusian or Atdabanian) Xihaoping Member of the Dengying Formation from Xiaowan, Xixiang County, southeastern Shaanxi Province, China are described herein. The first of these is a cap-shaped fossil we describe as Cambroskiadeion xiaowanense new genus and species. On its concave surface it bears a spine, the base of which is covered with numerous hemispherical verruculae. The long spine indicates that this was a sclerite rather than a univalved shell, although it remains unclear from what sort of animal it came. Similar fossils have been hypothesized to be halkieriid valves; although the rarity of halkieriid sclerites in the present samples argues against this view, it is possible these fossils are part of a similar multi-element skeleton. The second fossil is Acidocharacus longiconus Qin and Ding, 1988; it is known only from the Xihaoping Member and consists of a tall spine, often bearing barbs or bumps, attached to a rounded conical base. The base is covered with verruculae similar to those found on Cambroskiadeion. The function of these elements, and whether they were internal or external, remains unknown.

Cambrothyra ampulliformis Qian and Zhang, 1983, is a jar- or vase-shaped fossil known from the Lower Cambrian of Shaanxi and Hubei provinces, China. It has been interpreted as a protistan test or cyst or a metazoan sclerite. A large collection of specimens from the Xihaoping Member of the Dengying Formation in southern Shaanxi Province permits its detailed redescription. These fossils are highly variable in shape but this variation is continuous and does not support the current recognition of multiple species for this material. They were originally hollow with a restricted basal foramen and a calcareous wall probably composed of fibrous aragonite. All of these features support the identification of Cambrothyra as sclerites of a coeloscleritophoran, a problematic group of Cambrian scleritome-bearing metazoans. Furthermore, the walls of Cambrothyra sclerites contain numerous pores, a feature shared with other coeloscleritophorans. Cambrothyra resembles chancelloriids in particular due to the shared presence of a verruculose texture around the foramen and the absence of mirror-image pairs of asymmetric sclerites. However, unlike chancelloriids, which have rosette-like compound sclerites, the scleritome of Cambrothyra was dominated by isolated sclerites, with only a few pairs and clusters of sclerites and twin sclerites. Consequently, we hypothesize that Cambrothyra forms a clade with other chancelloriids, but represents a basal lineage that plesiomorphically retained isolated sclerites. The morphology of Cambrothyra sclerites, which shares features with both chancelloriids and halkieriids, thus supports the hypothesis that all coeloscleritophorans form a natural group.

Thalassemia Major (Cooley Anemia)
Eighty years ago, Dr. Thomas Cooley and his colleagues at the Children's Hospital of Michigan administered blood transfusions to children with a newly recognized clinical entity whose features included severe anemia, splenomegaly, and peculiar facies. Five years later, Cooley's name had become indelibly associated with the disease, and transfusions were administered along with ferrous carbonate, ultraviolet rays, and extract of pituitary gland. Not surprisingly, given the state of crossmatching at the time, the response to transfusion was poor. Another 30 years elapsed before improvements in blood banking and recognition of the benefits of a higher hemoglobin level came together to initiate the era of modern transfusion therapy for thalassemia major.
Transfusion Programs
In 1963, the results of an evaluation of 35 children with thalassemia major, aged 12 years or younger, whose pretransfusion hemoglobin levels fell into three categories – 4.0–5.9 g/dL, 6.0–7.9 g/dL, or 8.0–9.9 g/dL – were described. Children in the highest hemoglobin group had better linear growth, less enlargement of the liver and spleen, less facial and skull bony abnormalities, fewer fractures, and less cardiomegaly than children in the two lowest hemoglobin groups. Two patients who received regular red cell transfusions to maintain their hemoglobin level above 10 g/dL at all times were also reported. More than 40 years later, this regimen – hypertransfusion – remains the standard of care for the treatment of thalassemia major. General guidelines for hypertransfusion therapy are presented in Tables 29.1 and 2.

Context.
Healthcare data suggest that the incidence and severity of Clostridium difficile infection (CDI) in hospitals are increasing. However, the overall burden of disease and the mortality rate associated with CDI, including the contribution from cases of infection that occur in nursing homes, are poorly understood.
Objective.To describe the epidemiology, disease burden, and mortality rate of healthcare-onset CDI.
Methods.In 2006, active public reporting of healthcare-onset CDI, using standardized case definitions, was mandated for all Ohio hospitals and nursing homes. Incidence rates were determined and stratified according to healthcare facility characteristics. Death certificates that listed CDI were analyzed for trends.
Results.There were 14,329 CDI cases reported, including 6,376 cases at 210 hospitals (5,217 initial cases [ie, cases identified more than 48 hours after admission to a healthcare facility in patients who had not had CDI during the previous 6 months] and 1,159 recurrent cases [ie, cases involving patients who had had CDI during the previous 6 months]) and 7,953 cases at 955 nursing homes (4,880 initial and 3,073 recurrent cases). After adjusting for missing data, the estimated total was 18,200 cases of CDI, which included 7,000 hospital cases (5,700 initial and 1,300 recurrent cases) and 11,200 nursing homes cases (6,900 initial and 4,300 recurrent cases). The rate for initial cases was 6.4-7.9 cases/10,000 patient-days for hospitals and 1.7-2.9 cases/10,000 patient-days for nursing homes. The rate for initial cases in nursing homes decreased during the study (P < .001). Nonpediatric hospital status (P = .011), a smaller number of beds (P = .003), and location in the eastern or northeastern region of the state (P = .011) were each independently associated with a higher rate of initial cases in hospitals. Death certificates for 2006 listed CDI among the causes of death for 893 Ohio residents; between 2000 and 2006, this number increased more than 4-fold.
Conclusion.Healthcare-onset CDI represents a major public health threat that, when considered in the context of an increasing mortality rate, should justify a major focus on prevention efforts.

We have presented a case of folie à deux in which the wife was the primary (dominant) partner and the husband the secondary (submissive) partner. The husband's symptoms seemed to have resolved as his dependency shifted from his wife back to his mother. Treatment is recommended by deliberately shifting dependencies only when there is advantage in doing so. For most patients, the aim should be independence.

The behavior of AgReO4 under pressure has been studied in a diamond anvil cell. The equation of state determined by synchrotron radiation shows considerable change in compressibility with increasing pressure. X-ray and Raman scattering show a first-order phase transition from the scheelite to an as yet undetermined structure at 13 GPa. Changes in both the internal and external Raman active modes suggest a change in Re coordination. Optical transmission experiments demonstrate that the energy gap in the scheelitephase decreases at the rate of 75 meV/GPa. There is a small jump in the energy gap at 13 GPa, probably related to the structural transition. AgReO4 remains aninsulator to at least 47 GPa.
